2008 Fall Sports Registration:

A 2008 Family Membership, Fall Sports Form and the County Waiver Form must be completed.  Payment methods include check or credit card (Visa and Mastercard). 

All three forms and payment can either be:

1. Turned in during the noted registration times,
2. Dropped off at the MVAA office during regular office hours,
3. Sent via the mail to MVAA, 31 West Main, Suite A, Middletown, MD 21769

Late Fee $25.00 per family postmarked

2008 Family Membership Form *

For all fall participants, a 2008 Family Membership, the 2008 Sports registration form and the County Waiver are required.  Families with more than one child participating in a spring sport are only required to turn in one family membership form.

*If your family members participated in a Spring 2008 sport, the family membership is not required.  The family membership is valid for the entire calendar year.
