can be mailed in or dropped off at the MVAA office during
regular business hours.

2010 Family Membership Form
2010 Family
Membership & the 2010 Sports registration form are
required. Families with more than one child
participating are only required to
turn in one family membership form.
2) NOTE:
Frederick County Field Waivers must be completed for
each adult and player for all sports except In-House
soccer and Field Hockey. Please print and
complete/sign the forms below and mail in the signature
pages only. This is required for participation on
Frederick County fields.
