from 9am to Noon at the MMS Cafeteria.

Please register on-time!
Registrations are considered late with $25 late fee assessed after May 28th, 2011.
Registrations can be mailed in or dropped off.
To Register for Fall Sports, please
follow the 3 steps below:
1)
2011 Family Membership Form
REQUIRED for all families who DID NOT participate in a Spring 2011 sport. Families with more than one child participating are only required to turn in one family membership form.
2) Frederick County Field Waivers
REQUIRED for each player and adult (parents & coaches) for all sports except Cheerleading & In-House Soccer. Please print and complete/sign the forms below and mail in the signature pages (page 2) only. This is required for participation on Frederick County fields.
