Fall Sports Registration

Fall Sports Registration is now open

Baseball * Cheerleading * Football * Soccer *  Softball

 

 

Registration for fall sports to be held  May 12th & May 19th from 9 a.m. - noon

at the Middletown Middle School cafeteria.

*Those participating in cheerleading must attend one of the two registration dates for additional information.**

**Those participating in football must attend one of the above dates for fitting of equipment. **


LATE FEE of $25 required for all registrations postmarked after May 26th, 2012

Please follow the three steps below for registration:

 

1.)  Complete the Family Membership 2012 Form for all those who did not participate in a spring sport and have already paid this fee.

       This form and fee is REQUIRED for all athletes participating in 2012 MVAA sports.

 

2.)  Please complete the registration form for the desired sport below:

 

Baseball Fall 2012 Registration form

3.)  Please complete the following waiver forms required by  Frederick County Parks and Recreation:

 Adult Waiver Form FCP & R

Youth Waiver Form FCP & R
